GRANDPARENT FILINGS

GRANDPARENT CARETAKER AFFIDAVIT - used when parents' whereabouts are unknown.

GRANDPARENT POWER OF ATTORNEY - used when parent(s) are available.

Beginning in July, 2004, grandparents may file paperwork to have physical custody of their grandchildren.  The paperwork assists grandparents in enrolling the grandchild in school and obtaining medical, dental and psychological treatment for him or her.   THIS DOES NOT GIVE THE GRANDPARENT LEGAL CUSTODY.

In December, 2012, some updates to the law occurred and new forms reflecting these changes are being developed.  As Juvenile Court Staff cannot assist in completing the paperwork, please contact the Kinship Caregiver Coalition and schedule an appointment at:  937-775-8245 (Intake), 937-775-8250 and 937-775-8247 (Kinship Navigators).  The Kinship Navigators can assist with these forms and identify other resources for grandparents.
